Recognizing Poison Plants

Identifying the poison ivy plant is the first step in avoiding the rash. The popular saying “leaves of three, beware of me” is a good rule of thumb for poison ivy and poison oak but is only partly correct. A more exact saying would be “leaflets of three, beware of me” because each leaf has three leaflets. Poison sumac, however, has a row of paired leaves. The middle or end leaf is on a longer stalk than the other two or more leaves. This differs from most other three-leaf look-alikes.

Poison ivy has different forms. It grows as vines or low shrubs. Poison oak, with its oak-like leaves, is a low shrub in the East and can be a low or high shrub in the West. Poison sumac is a tall shrub or small tree. The plants also differ in where they grow. Poison ivy grows in fertile, well-drained soil. Western poison oak needs a great deal of water, and eastern poison oak prefers sandy soil but sometimes grows near lakes. Poison sumac tends to grow in standing water such as peat bogs.

These plants are common in the spring and summer. When they grow, there is plenty of sap and the plants bruise easily. The leaves may have black marks where they have been injured. Although poison ivy rash is usually a summer complaint, cases may occur in winter when people burn wood that has urushiol on it or cut poison ivy vines for wreaths.

It is important to recognize these toxic plants in all seasons. In the early fall, the leaves can turn colors such as yellow or red when other plants are still green. Other symptoms include cramp and pain in the stomach and pain when passing stool. Onset of Ulcerative Colitis is typically between the ages of 15 and 35. In the Western world, it is found at a frequency of about 200 per 100,000 persons. Crohn’s on the other hand can affect any part of the gastrointestinal tract from the mouth to the anus. Crohn’s disease is also associated with abdominal pain and diarrhoea which may be bloody. Common symptoms of Crohn’s include vomiting and weight loss. Unlike Colitis, Crohn’s is often associated with additional complications outside of the gut such as skin rash, arthritis, uveitis, and tiredness.

Role of HLA Genes

The most consistently found association between a gene and Ulcerative Colitis is with the HLA class II allele HLA-DRB1*01:03. The association is particularly strong in patients with severe disease who end up needing to have their Colons surgically removed in a process known as Colectomy. Even though not all patients who end up needing Colectomy have the HLA-DRB1*01:03 allele, presence of the HLA-DRB1*01:03 gene tend to be associated with a shorter mean time to surgery. Another HLA class II allele shown to be associated with Ulcerative Colitis in some populations though the strength of the association varies in different populations is HLA-DRB1*15:02. Genes which are thought to be protective against development of Ulcerative Colitis include HLA-DRB1*15:01 and DRB1*04.

The most consistently found association between a gene and Crohn’s disease is with the HLA class II allele HLA-DRB1*07. The association is particularly strong in patients with Crohn’s affecting the ileum. However more recently, HLA-DRB1*01:03 which is associated with Colitis has also been shown more recently to be associated with Crohn’s. As with ulcerative colitis, patients the HLA-DRB1*01:03 gene may have more severe disease needing Colectomy. Like Colitis, HLA-DRB1*15:01 appears to confer protection against Crohn’s.

Role of Non HLA Genes

The success of scientists in sequencing the whole of the genome has opened up new avenues to explore the association of a number of other non HLA genes with Inflammatory Bowel Diseases. Candidate genes which have been studied for association with Ulcerative Colitis include the Interleukin-23 Receptor (IL23R). Of particular interest in Crohn’s patients are the genes for the pattern recognition receptors that recognise bacteria. These include ‘Nucleotide-binding Oligomerisation Domain containing 2′ / ‘Caspase Recruitment Domain family, member 15′ (also known as NOD2/CARD15).

Value of Genetic Testing In Inflammatory Bowell Diseases

The association between HLA and Inflammatory Bowel Disease including Ulcerative Colitis and Crohn’s disease is of low specificity and sensitivity, which limits the value of HLA genetic testing for diagnostic purposes in these conditions. Presence of HLA-DRB1*01:03 may be of value in predicting patients who may potentially require Colectomy so that for these patients, early more aggressive treatments can be considered to try to save the Colon. The non HLA genes may hold more promise for genes with diagnostic value for Inflammatory Bowel Disease but its early days and more research is needed.
